Your Patient will qualify for care if:

- They are compliant with their doctor's visit schedule .
- They have a plan of care.
- They have difficulty moving or leaving home unassisted.
- They are in need of intermittent skilled Nursing Care, Physical/Occupational/Speech Therapy or other skilled medical services.
