You qualify for Home Care if:
- You are under the care of a Physician
- You have a Plan of Care developed by your Physician
- You have a difficulty leaving home unassisted
- You are in need of intermittent skilled Nursing Care, Physical/Occupational/Speech Therapy or other skilled medical services.

How much will it cost the patient?

• All services for persons who are Medicare beneficiaries will be 100% covered under the Medicare Program. This means that there is NO OUT OF POCKET COST to YOU or YOUR FAMILY, if all the required Medicare conditions to qualify, as stated above are satisfactorily met.
• Foreign speaking patients and patients with limited English proficiency will be offered a Translator at No Cost to the patient. Please contact our office to request for this service as required.
• Sensory Impaired Patients will be provided with a qualified sign language interpreter and/or other auxiliary aides at No Cost, as required.
